Where do you get your information from? How old is the information posted? I know that your information is outdated as I am the careplan coordinator for Villa Healthcare East in Sherman IL. The facility is completely restraint free, thus the information cannot be correct. Also, how long ago were the definciencies updated. The last survey only had 4 tags, none of which resulted in actual harm. It is possible that your information needs to be updated. I am aware of a bad survey prior to my arrival at the facility over 1.5 years ago. Please make sure to correctly reflect the level of care that we are providing. Thanks
