How do I go about getting a copy of my medical records from my last hospitalization in 2010. I recently suffered a heart attack and was taken to Harbor UCLA and have now been referred to their Cardiology Clinic for follow-up care and the doctor indicated that it was necessary to obtain a copy of my medical record.
